2. ВВЕДЕНИЕ
Инсульт является одной из основных причин инвалидности и смерти населения в мире (Roger et al., 2010).
Инсульты подразделяются на геморрагические, при которых происходит кровоизлияние в мозг, и ишемические, при которых происходит нарушение кровоснабжения в определенной области головного мозга и нейроны находятся в состоянии ишемии с последующей реперфузией. Подавляющее большинство инсультов являются ишемическими (Donnan et al., 2008).
При ишемическом инсульте выделяют три зоны в пораженной области мозга: ядро ишемии, пенумбру и зону сниженного кровотока. В ядре клетки погибают из-за недостатка кислорода и глюкозы. Клетки зоны сниженного кровотока не погибают, так как уровень недостатка глюкозы и кислорода в них не является критическим. Пенумбра, или зона ишемической полутени, является зоной, в которой клетки находятся в пограничном состоянии между выживанием и гибелью, так как в них одновременно запускаются проапоптотические и антиапоптотические сигнальные каскады, сверхэкспрессируются про- и антиапоптотические белки, и баланс между ними определяет судьбу нейронов (Donnan et al., 2008).
Наиболее значимыми событиями, приводящими к гибели нейронов, являются глутаматная эксайтотоксичность, обусловленная накоплением глутамата во внеклеточном пространстве, активацией рецепторов глутамата и увеличением концентрации внутриклеточного кальция; повреждение митохондрий; окислительный стресс, обусловленный высоким уровнем активных форм кислорода (АФК) (Cerio et al., 2013). Также ишемический инсульт сопровождается воспалением и реперфузионным повреждением, то есть при восстановлении кровотока уровень кислорода повышается, что приводит к образованию еще большего количества АФК и снижению эндогенного антиоксидантного потенциала (Wu et al., 2018).
Для изучения патогенетических механизмов ишемического инсульта и поиска перспективных нейропротекторных препаратов в экспериментах in vitro нейроны помещают в условия глюкозо-кислородной депривации, ингибируют электрон-транспортную цепь митохондрий или моделируют глутаматную эксайтотоксичность (Holloway, Gavins, 2016), а также воспроизводят ишемический инсульт in vivo путем моделирования фокальной ишемии головного мозга, индуцированной окклюзией среднемозговой артерии (Kleinschnitz et al., 2015).
На сегодняшний день внутривенное введение рекомбинантного тканевого активатора плазминогена является наиболее приоритетным лечением острого ишемического инсульта (Adams et al., 2007; You et al., 2018; Rabinstein, 2020), однако его рекомендовано применять в течение 60 минут после прибытия пациента в больницу для снижения риска тяжелых последствий и смертельного исхода (Fonarow et al., 2011). Терапевтическое окно для применения рекомбинантного тканевого активатора плазминогена составляет 4,5 часа, что актуально всего для 5% пациентов (Kleinschnitz et al., 2015).
Антиагрегантная терапия является ключевым фармакологическим подходом к профилактике острого ишемического инсульта (Donnan et al., 2008; Fiolaki et al., 2017). При этом наиболее широко используемым антиагрегантным препаратам является аспирин (ацетилсалициловая кислота) (Fiolaki et al., 2017), эффективность которого при назначении в острый период ишемического инсульта подкреплена данными доказательной медицины (Chen, 1997). Однако, применение аспирина может привести к негативным побочным эффектам со стороны ЖКТ (Haastrup, Jarbøl, 2014).
Таким образом, актуален поиск препаратов нейропротекторного действия, обладающих как антиоксидантными, так и антиагрегантными свойствами для профилактики и лечения ишемического инсульта, а также предотвращения повторного инсульта. В то же время, важным аспектом эффективности такого препарата является его устойчивость и отсутствие негативных побочных эффектов.
